Submitting a Technical Request won't work

We have issues with one of our Silhouette licenses and are therefore trying to submit a Technical Request using this form:
It seems the form isn’t working though - after hitting ‘Submit’ we get forwarded to a ‘blank’ page - with no information on the success of the request.
(It’s also not showing up in the ‘Support Cases’ section of the profile page yet)
